Monitoring And Evaluation

The monitoring and evaluation functions are coordinated by the Steering and Quality Improvement Committee and are conducted through:

Internal and External Customer satisfaction surveys Employee

satisfaction surveys Facility reviews, surveys and Quarterly Master Audit Monitoring of trends and patterns in incident reporting, recipient rights and person-centered planning initiatives Demographics of persons served

The standing committees of the Steering and Quality Improvement program are comprised of administrative, facility managers, health and safety coordinators and direct care staff. Appointments to the aforementioned committees are recommended by the Steering Committee and approved by the Christ Centered Homes Executive Director.

Utilization and Outcomes Committee: The committee reviews the use of resources to insure that quality care is provided in a cost-effective manner. Trends are monitored to assure appropriateness and efficacious use of residential services. The monitoringfimction includes review of admissions, discharges and aftercare, quality of life, and consumer satisfaction.

Credentialing Committee: This committee provides oversight of the organizations credentialing and recredentialing process. The committee monitors performance with identified indicators and assimilates information to facility programs. Staff training needs are identified and recommendation are forwarded to the Steering and Quality Improvement Committees.

Health, and Accessibilily Management Committee: The committee monitors physical plant for accessibility, sanitation, and infection control compliance. It also measures, monitors and evaluates for efficiency safety compliance with established policies and procedures. Furthermore, the committee trains facility staff for consistent effectiveness.

Customer Advisojy Council: This council is comprised of persons served, family members, guardian and individuals invested in their lives. These are the primary spokespersons for consumer-related suggestions, concerns or improvements. Input received is presented to the Steering Committee for review and assignments to other committees to coordinate. Meetings are held quarterly, minutes are recorded and remain on file.
